  • I am calling to verify my insurance benefits. What level of coverage for outpatient mental health counseling do I have?
  • Are there any services or treatment protocols excluded in my benefits?
  • What is my deductible (the amount you pay out of pocket before your insurance benefits begin)?
  • Has my deductible been met?
  • Are there diagnoses that this policy will not cover?
  • How many sessions per calendar (or policy) year does my plan cover?
  • How many sessions are remaining for this calendar year?
  • Is Dr. Cassil on the provider panel? If not, how much will my benefits cover for an out-of-network provider?
  • Is a referral from my primary care physician required? Or if you are a student, Is a referral from my school's health and counseling center required?
